Appointments may be scheduled either by the patient, emergency department or by the patient's primary care physician, depending on the requirements of your insurance plan.

If the injury is covered under Worker's Compensation Insurance, we encourage that the employer call to verify the injury and insurance information prior to scheduling an appointment.

To schedule and appointment at one of our 5 locations, please call Central Scheduling at 724-225-8657 or 1-800-828- CAST (2278) Monday through Friday between the hours of 8am and 4:30 pm.

Please be prepared with the following:

• Insurance Information
• Home address and Phone number
• Nature of your injury
