How to Prepare for Your Visit
On your arrival to Advanced Orthopaedics, please sign in at the registration window and have a seat. If this is your first visit you will be asked to provide registration information, insurance information, insurance card, and photo identification. If you have visited our office in the past the receptionist will confirm your information and make any updates or changes as needed. Please remember to bring your insurance card on EVERY visit.
Listed below are important details to be aware of that will help you be more prepared for your visit:
- It is important to bring your current insurance card (s) and photo identification with you to each and every appointment.
- If you have a child insured by a parent or guardian not living in the home, please obtain the insurance card or a copy ( front and back) of it in advance and bring it with you. We will also need the responsible party's information including address, date of birth, and SSN.
- Patients under the age of 18 MUST have a parent or guardian present for the visit.
- If you have previous x-rays or diagnostic tests pertinent to the injury being evaluated, please bring those image reports with you. This will prevent duplication of testing and unnecessary charges that may or may not be covered by your insurance plan. Scans or X-rays may be supplied to you on film or CD.
- Bring a list of all current medications, including over the counter medicines and vitamins.
- Coordinate transportation in advance to plan accordingly to arrive at your appointment on time. Also notify the office as soon as possible if you are unable to keep the appointment time that you have booked.
- If you are diabetic or have a health condition that requires frequent nourishment, you are encouraged to bring a snack. There is no cafeteria available at Advance Orthopaedics.
- Please be prepared to pay all co-payment at each appointment. If you do not have insurance, please be prepared to make a payment in full. You are welcome to speak with our billing department directly to inquire about fees, payment plans, etc.
- If your care is going to be covered by a third party liability coverage it is critical that you bring all the pertinent information with you. This includes :
- Claim number
- Payer's Name, Address & Phone Number
- Payer's Insurance Name, Address, & Phone Number
- Contact Person regarding your claim
- If you do not speak English, please invite a bilingual speaking adult (over 18 years of age) to attend the appointment with you. Advanced Orthopaedics does not have interpreters on staff.
- If you injury is work related, pre-authorization is required before an appointment is scheduled. If you report the injury as a worker's compensation claim during your visit and your appointment was not pre-authorized, you are responsible for the fees.
- We encourage you to come prepared with questions and concerns. Feel free to bring a pen and paper to take notes during your visit to help you remember everything that you will learn.
- Please know that the appointment time provided to you is not necessarily a confirmation when you will see the physician. The appointment time is the time we have asked that you be available so we can complete the following in order to prepare for your exam with the physician: Registration, Necessary X-rays, Review of History, Casting / Bracing and/or a Surgical Evaluation.
- We realize the value of your time. Our Physicians try to stay on schedule for each appointment. However, due to the nature of orthopaedic problems, some delays may occur, as some patients present more complex or urgent problems requiring an extended evaluation. We thank you for your understanding and patience if this should occur. Feel free to call ahead to check and inquire if your physician is currently on schedule.
- If you are unable to keep your scheduled appointment, please notify us so that we may use that time for other patients.
