• If you are in need of a copy of your Medical Records, please allow 7 - 10 business days for processing. Please fill out the attached form and either mail, fax, or drop off the form in one of our 5 locations.
• If you need a copy of your x-rays you can call to arrange a pick up at one of our 5 locations. Please fill out the form attached in order for us to release the X-rays.
  The fees for x-rays are as follows:
• Actual Film: $20 deposit which will be refunded by check after the X-rays are returned to our office.
• Copies of Actual film: $5 per page of film non-refundable charge (this may take up to 10 business to process)
• Digital Film: $5 non-refundable charge
• Form Completion takes 10-14 days and may require an up-front charge of $5 per page before they are completed.
